Extract only the decisions from a meeting
Keeps decisions rather than the whole record — who decided what, when, and on what basis, in a form you can search later.

Extract the decision log from this meeting record. For each decision, one row: - What was decided (one sentence, in the affirmative) - Who decided it - The stated basis - What it supersedes, if anything - When it takes effect Rules: - *A decision is something that closes an option.* Discussion, agreement to look into it, and "we should probably" are not decisions — list those separately under "not decided". - If the record does not state a basis, write "basis not recorded". *Do not reconstruct the reasoning.* That is the single most dangerous thing to get wrong in a decision log. - Keep the decider's own wording where it is unambiguous. - End with decisions that contradict each other, if any.

What this prompt does
What bites months later is "why did we decide this?" Full minutes are too long to revisit. This pulls out decisions with their stated basis, and marks "basis not recorded" when the record does not contain one.
